Registration Forms

Everything you need to get your student ready for Zealous.

Before attending any Zealous Youth Ministry event or conference, each participant needs a completed release form on file. Please download the applicable form, fill it out, and sign it. You can bring the signed form to the event or send it back to us ahead of time — whichever’s easiest for you.

Which form do I need?

• Under 18? Download the Minor Participant Release Form. A parent or legal guardian will need to complete and sign it.

• 18 or older? Download the Adult Participant Release Form and sign it yourself.

How to return your form

1. Download and print the correct form.

2. Fill it out completely and sign it.

3. Bring it with you to check-in, or scan/photograph it and email it to us at zealousyouthconference@gmail.com before the event.
